SNAP Community Partners
Overview
LDH SNAP Community Partners is a collaborative network of Community-Based Organizations (CBO) that serve as:
SNAP application assisters who guide individuals through the application process (depending on selected partner level).
Resource centers for information and materials related to Economic Stability programs and services.
Gold Level
As a Gold level partner, responsibilities include all requirements listed for the Bronze and Silver levels, plus the following:
Have partner staff available to complete a paper or online application on behalf of the applicant upon request.
Silver Level
As a Silver level partner, responsibilities include all requirements listed for the Bronze level plus the following:
Make at least one computer available with internet access for individuals to enter their own information into the online application and have partner staff available to answer questions during the process.
Bronze Level
As a Bronze-level partner, responsibilities include the following:
Make informational materials and resources available to individuals, such as SNAP paper applications and instructions, program brochures and pamphlets, and prepaid Document Processing Center (DPC) envelopes. These materials will be provided to you at no cost by the agency.
May offer a computer, if one is available for public use, for individuals to enter their own information into the online application. It is not necessary for partner staff to be available to answer questions.
Offer a prepaid, self-addressed, stamped DPC envelope to individuals to mail paper applications, verification documents, etc., to a local LDH Economic Stability office.
Provide individuals with a place that affords sufficient privacy to complete paper applications (and online application for Silver and Gold partner levels) without compromising confidential information.
May make other office equipment, such as telephones, fax machines, copiers, etc., available for individuals to carry out LDH Economic Stability program business.

SNAP Community Partners
do not
determine eligibility for LDH Economic Stability program benefits. Program staff will be responsible for processing all applications completed at a SNAP Community Partner site. This includes conducting case interviews, determining eligibility for benefits, notifying individuals of decisions, and all subsequent casework.

CEU staff will consider all submissions, interview potential partners, and determine partnership suitability for all interested organizations.
After becoming a SNAP Community Partner, a Community Engagement consultant will coordinate a visit to provide you with signage/posters, a window decal, pamphlets and brochures, paper applications, and DPC envelopes.
