Our interdisciplinary team cares for those who have had heart failure, heart surgery, or any disease, illness or injury that may have affected your heart or for those who need preparation for a Left Ventricular Assist Device (LVAD), implantation or care for their LVAD after being stabilized in an acute care setting. We treat your complex cardiac needs with the goal to reduce hospital readmissions, increase your independence in self-management and improve your quality of life at home and in the community.
