Pediatric Complex Care Resource Centers | Department of Human Services | Commonwealth of Pennsylvania
Pediatric Complex Care Resource Centers (PCCRC)
The Department of Human Services’ Offices of Medical Assistance Programs (OMAP) and Development Programs (ODP) have developed a system of supports to better serve children and adolescents living in the community who receive pediatric home health care (in-home shift nursing or home health aide services) by supporting their families, caregivers, and healthcare providers.
A key component of developing this system of supports is improving the relationship between the home health care providers and families.
Pediatric Complex Care Resource Centers (PCCRCs) were created to provide education, training, and non-medical support to caregiver teams to assist with building relationships and co-navigating the various systems and resources that are available to support them.

PCCRC Journey Maps
Journey Map for Caregiver Milestones
The PCCRC Caregiver Journey Map is organized around typical milestones a family may experience, and it describes what caregivers often need at each stage.
Journey Map for Collaborative Agencies
The PCCRC Collaborative Agencies Map shows the network of partners that support families across the journey and how those agencies connect to PCCRC and to each other.
PCCRC Resources
Pediatric Coaches provide personalized support to caregiver teams to strengthen collaboration around the child’s needs through advocacy, communication support, and targeted education and training.
Family Facilitators support children and adolescents with complex medical needs as they transition from facilities and hospitals to community settings. The Family Facilitator also supports the diversion of placements from community to congregate care settings.
The Online Training Hub offers self-paced learning courses on various home-care topics designed for all members of the caregiver team and information on supports and resources are available through MyODP.
