About Hospice Care
Hospice is a holistic, whole-person approach in caring for terminally ill people who wish to concentrate on their comfort and quality of life. It is for those patients that no longer seek aggressive treatment.
Hospice isn't a place - it's a concept of care that can be provided anywhere. In fact, most of the care provided by the Summa Health Hospice team is provided in the comfort of the patient’s home, nursing home or assisted living facility.
Hospice care involves a team of trained professionals, available 24 hours a day, which is focused on ensuring that the patients’ needs and wishes are met. The hospice team will provide medical attention, symptom management, emotional and spiritual support, and personal care, tailored to an individual's needs and wishes. The Summa Health Hospice team will work with the patient and their loved ones, offering compassion and education, walking alongside them in their journey.
Our goal is to provide uncompromising quality care to terminally ill patients and the people who love and care for them. Our hospice services allow patients and their families to make the most of every day, while we help to fill in the gaps, with individually designed patient care plans and a supportive network of medical professionals and volunteers.
We offer hospice services in many northeast Ohio counties including Summit, Medina, Portage, Wayne and Stark counties. We offer inpatient hospice care at Summa Health System - Akron and Barberton Campuses.
Patients who are eligible for hospice care are:
Patients who have been diagnosed with a terminal illness and have a life expectancy of months rather than years.
Patients who can no longer benefit from curative care and have agreed to a comfort-oriented philosophy of treatment.
Patients and families that have agreed to care that is primarily home-based. "Home" may include the patient's home, a caregiver’s home or residential care facility (nursing home).
Hospice Services
Summa Health Hospice believes in treating the whole patient, mind, body and soul. These disciplines make up the team that surround the patient and their family and work closely with your personal physician:
Physician/nurse practitioner
Pharmacist
Nurse- Registered and Licensed Practical Nurses
Home health aide
Social worker
Chaplain
Massage therapist
Grief resources
The team also works closely with Volunteer Services.
Summa Health Hospice volunteers only know one way of giving – with both hands, with all their hearts and without a second thought. Our Summa Health Hospice volunteers assist our staff by providing specialized programs and services to help patients and families, such as companion visits to combat loneliness, sitting vigil, offering respite care so loved ones can run an errand, Gifts of a Lifetime Legacy writers or Veteran Pinning, to name a few examples.
We also offer bereavement support and counseling. We understand that the grieving process can vary from person to person and there is no timeline on grief. Bereavement services are offered over the course of 13 months after the patient passes. These services include mailings with grief support information, phone calls, visits, grief support groups, memorial services and referrals to community grief support resources.
