Talk to your healthcare team about your concerns.
If the healthcare team is unable to resolve your concerns, ask to speak with a manager where you’re receiving care.
If your concerns remain unresolved, please contact Patient Relations at 503-681-4357 (HELP) for further assistance.
Our Patient Relations team is available to facilitate a formal review and follow-up of your concerns. Information shared with us is reported to the
leadership team
, so we can ensure we’re providing exceptional care organization-wide.

Patients, family members, and visitors may talk with a Patient Relations representative at 503-681-4357 (HELP). Our representative is available by phone Monday – Friday from 8:00 a.m. to 4:30 p.m.
For urgent concerns after hours or on weekends, please contact the Administrative Nursing Supervisor at 503-681-1255.
