Informed financial consent
My Admission
You can complete your admission forms online for all planned admissions at St John of God Health Care hospitals using MyAdmission. This includes submitting financial consent forms, making payment and booking maternity appointments and classes.
Start or continue an online admission
To protect your sensitive health information and keep your data safe, multi-factor authentication (MFA) is in place to help us confirm your identity in a secure way and is used in addition to your username and password.
When logging in, you’ll be prompted to verify your identity via a code sent to your email or mobile phone.
If you’ve previously registered an account with us, you will be prompted to set this up next time you log in.

How to complete your admission
Before you start your online admission registration, please make sure you have the following information to hand:
The reason for your admission including the name of any procedure.
The date of your admission/procedure.
Whether you are having a same day procedure or overnight stay.
If applicable, you will also need:
Medicare card
private health fund information e.g. membership card
a list of current medication
medical history
pharmacy safety net, concession card or DVA Gold Card.
You will need a valid email address and mobile phone number to register. If you do not have both an email address and mobile phone number please contact us on the number below.
The registration will take approximately 15 to 45 minutes to complete, depending on your medical history. If you do not have time to complete the form in one sitting, you can save it at any time by clicking on the save icon to continue at a later time.
Note: If this is your first time completing an online admission registration, you will need to click on 'Create User' when you get to the login page to register your details.
If you are a privately insured patient, it is essential that you check with your health fund to confirm your level of cover for your upcoming admission and understand any out-of-pocket expenses you may be required to pay.
As part of your admission, you'll need to acknowledge your informed financial consent (IFC) form.

For family members and support people
You are welcome to assist a patient to complete their admission registration online. To complete the online registration, you will need to get the following information from the patient:
Next of kin details
Medicare card
Private health insurance details
Current medications
Knowledge of medical history
Steps:
Register with a valid email and password. A mobile phone number is required for authentication purposes and also to receive hospital reminders in the lead up to the patient’s admission.
Complete the admission details section, consisting of basic demographic information. Allow 10 to 15 minutes.
Complete the clinical history section, consisting of past surgeries, current allergies and medication. Allow 10 to 30 minutes, depending on medical history.
