There are also more than 350 individual and group dental practices. To become a patient or an employee of any of these practices, follow one of the links below or do a simple online search to find contact information for the practices of interest. Many of these practices are Federally Qualified Health Center (FQHC) sites or Rural Health Clinics (RHCs), and are considered part of the health care safety net. Under Medicare and Medicaid rules, safety net providers are required to provide health care services to all Vermonters, regardless of insurance status and mostly with a sliding fee scale. Most practices in Vermont include a mix of physicians, physician assistants and nurse practitioners, and are certified patient-centered medical homes. Find a health center near you Rural Health Clinics Ten Rural Health Clinics (RHCs) provide health care in Vermont, most in conjunction with local hospitals. Independent practices located in areas that meet certain criteria can become a Rural Health Clinic and receive enhanced reimbursement for their Medicare and Medicaid patients. More information at: Rural Health Clinics CMS Vermont's Free and Referral Clinics Eleven free clinics work with partners throughout the state to help uninsured and under-insured Vermonters get the medical and dental care, medications and insurance coverage they need when they need it. Most are members of the Vermont's Free & Referral Clinics (vtfreeclinics.org) Several clinics have regular office hours when you can be seen by a doctor, nurse or other provider. All clinics will help you find a regular health care provider and enroll in Medicaid or a private health insurance plan through Vermont Health Connect.
