Documents to be submitted:
formal letter of application
ASI Form No. 6 (application form) two (2) copies
Official Transcript of records (Original copy with Special Order)
Foreigners should have their transcript of records authenticated by the Philippine Embassy or Consulate in their country of origin or legal residence. “SEEN and NOTED” stamp is not acceptable.
diploma or certificate of completion of any bachelor’s degree
two (2) letters of recommendation from the agency or organization the applicant is connected with
short autobiography (see below for the guide)
3 passport-size pictures (taken within 6 months) during application period
passed the entrance test
passed the interview

Additional requirements:
Personal History Statement (Revised Form 1998) Six (6) Copies
Xerox copy of Passport pages where name, photo, birthdate, and birthplace appear
A notarized affidavit of support AND proof of adequate financial report to cover expenses for student’s accommodation and subsistence, school dues and other incidental expenses.
For scholars, certificate of scholarship from the institution concerned.
English language proficiency (oral and written)
To be submitted to the Philippine Embassy in your country when you pick up your student visa:
