The Health Information Management Department  (more commonly known as Medical Records) is available Monday thru Friday from 8:00 a.m. to 4:30 p.m.
The HIM Department includes Coders, Transcriptionists, and Clerks whose primary function is ensuring that your medical record is kept complete, accurate, and confidential. Our office complies with the HIPAA Privacy Standards and we will not release your Private Health Information (PHI) without your written authorization.
Do you have questions on how to obtain copies of your Medical Record or how to complete a Release of Information?  Call the H.I.M. Department directly at 217-784-2605.
Release of Information forms can be faxed to 217-784-2657.
If you have a HIPAA complaint you may contact the compliance line by dialing 217-784-2456.
