Certificate of Professional Limited Liability Partnership: Online Instructions
Purpose:
A Certificate of Limited Liability Partnership is used to create a new business entity that has not previously been registered with the Office of the Secretary of State; or is beyond its five (5) year reinstatement period.
Filing Fee:
$180
in addition to an online processing fee.
Expedited Service:
You may add expedited priority for an additional $100 at checkout, which is generally processed within three working days.

The name of a Professional Service Corporation organized to render dental services must contain the full names or surnames of all shareholders and no other word than "Chartered" or the words "Professional Services" or the abbreviation "P.S." or "P.C.". The name is limited to a maximum of 255 characters. If you wish to use a name longer than 255 characters, your application must be filed in paper form instead of electronically.
