Order Copies of Vermont Vital Records
The Department of Health issues certified and noncertified copies of vital records for births, deaths, marriages, and divorces that took place in the state of Vermont.
Certified copies
are printed on State engraved security paper with raised seal, costing $10 per certificate (plus a $2 processing fee if ordered online).
Only family members, legal guardians, certain court-appointed parties or their legal representatives can apply for a certified copy of a birth or death certificate. For death certificates, a funeral home or crematorium may apply for a certified copy.
Applicants must show valid identification when applying for a certified copy of a birth or death certificate.
Noncertified copies
are printed on plain white paper with a “noncertified copy” watermark and are available at no charge.
Birth and death records from January 1909 to present, marriages, civil unions, divorces, and dissolutions may be ordered from our office. Vital Records may also be ordered from the
Vermont State Archives and Records Administration
and from most Vermont Town/City Clerks.
Option One: Talk to your Town Clerk
If you are in Vermont, the
fastest
and
cheapest
way to get a copy of a vital record is to order it directly from your Town or City Clerk. They can issue both noncertified and certified copies of the following vital records:
Any birth or death certificate from January 1, 1909, to present.
Birth or death certificates from 1908 and earlier if filed in their office.
Marriage and civil unions certificates on file in their office.

You can order the following vital records by mail:
Birth and Death Certificates January 1, 1909, to present
Marriages and Divorces from 2024 to present, and Dissolutions of Civil Union (all years)
Marriages and Divorces from 2023 and Earlier, and Civil Unions (all years)
Follow the instructions on the application you choose and mail them to the address stated on the application.
Order noncertified copies of vital event certificates
To order noncertified copies of Birth or Death Certificates from 1909 to present, marriage and divorce certificates from 2024 to present, or civil union dissolutions for all years, please email

and include the following information in your request:
Type of event (birth, death, marriage, etc.).
Name(s) of the individual(s) named on the certificate.

Correct or amend a birth certificate

Application to Correct or Amend a Vermont Birth Certificate
, along with any required documents or supporting evidence.
Please note these special circumstances:
Completions or changes to the child’s name within six months of the date of birth requires the signature of both parents if two parents are listed on the birth certificate. Completions or changes to the child’s name more than 6 months after the date of birth requires a certified copy of a court order.
To add a second parent to the child’s birth certificate requires a certified copy of a court order adjudicating parentage or a completed
Voluntary Acknowledgement of Parentage Form
and, if applicable, a
Denial of Parentage Form
.

Change gender marker on birth certificate

Correct or amend a death certificate
A death certificate may be corrected within six months from the date of death or amended after six months from the date of death by application submitted by the certifying licensed health care professional, medical examiner, hospital, nursing home, or funeral director.
Correct or amend a marriage certificate
A marriage certificate may be corrected within six months from the date of marriage by application to the Town/City Clerk who issued the certificate. Amendments after six months from the date of marriage require a court order.
Delayed birth certificate
A delayed birth certificate may be issued for a person born in Vermont from whom no birth certificate was filed during the first year following the birth. A person, their parent or guardian may submit to Vital Records an

Beginning July 1, 2023, adoptees may
request a certified copy of their original Vermont birth certificate
from the Vermont Department of Health.

Order Copies of Vermont Vital Records
The Department of Health issues certified and noncertified copies of vital records for births, deaths, marriages, and divorces that took place in the state of Vermont.
Certified copies
