ENROLLMENT PROCEDURES
FRESHMEN
SDPC:
Take the College Assessment Test at the Student Placement & Development Center and wait for your results.
Requirements:
ONE 1×1 ID Picture
Senior High School Card or ID
Pencil with Eraser
Testing fee receipt from Accounting Office (P150.00)
COLLEGE DEPARTMENT:
Proceed to your college of choice and present the following
requirements:
Photocopy of Senior High School Card (SF9)
TWO Passport size pictures
SDPC Test Result
Students must have an interview with the Dean for Admission Slip.
REGISTRAR’S OFFICE:
Fill up Student Information Sheet (SIS) through the UNO-R Information System at the and submit the following
Requirements:
School Form (SF9): Senior High School Card (Original)
Original PSA/NSO Birth Certificate
Original PSA/NSO Marriage Contract (for married women only)
ONE 2×2 ID Picture with name tag
College Admission Test Result
Once your application is approved, you will receive accounting sheet indicating your
Student Personal Number (SPN).
Use your SPN for payment at the accounting office and for future transactions at the University.
ACCOUNTING OFFICE:
Pay the initial fee of
P1,500.00
upon registration and the mandatory fees. Student receives Official Receipt (OR).
DSA OFFICE:
Go to the Student Affairs Office, submit your Good Moral Character, and sign the Discipline Probation Contract.  Ask about uniform designs and policies.
SECURITY OFFICE:
Secure your School ID card from the Safety, Security, and Discipline Office (St. Augustine Hall, Ground Floor).
CONTINUING
ACCOUNTING OFFICE:
Pay initial fee of
P1,500.00
upon registration and the mandatory fees. Student receives Official Receipt (OR).
COLLEGE DEPARTMENT:
Department head evaluates student’s load. Enrolment officer encodes student’s enrolment details. Student receives hard copy of enrolment form.
SECURITY OFFICE:
Validate your student identification card.
Note:
Those who are under academic probation should see the Dean before making payment at the Accounting Office.
Those who are subject to clearance by the Office of the Director of Student Affairs should see the Director before making payment at the Accounting Office.
TRANSFEREE
ASSESSMENT (College Department):
Schedule an interview with the college dean to review your academic records and receive a recommendation for acceptance. Bring your academic documents, including your Transcript of Records (TOR). Fill out the Transferee Application Form (TAF) and have it signed by the dean.
DSA OFFICE:
Undergo an interview with the Director of Student Affairs (St. Ezekiel Moreno Hall, Ground Floor); the director signs the TAF to recommend you for College Assessment Test (CAT).
SDPC:
Present the recommendation from the DSA. Take the College Assessment Test at the Student Development and Placement Center and wait to claim your result.

Enrolment officer encodes student’s enrolment details. Student receives hard copy of enrolment form.
RETURNEE
REGISTRAR’S OFFICE:
Students gets confirmation and updates returnee’s status. Student receives returning student admission slip.
COLLEGE (Department):
Dean evaluates student’s previous record and recommends readmission.
DSA OFFICE:
Student verifies non-academic record.
SDPC:
Student updates cumulative record.
ACCOUNTING OFFICE:
Pay initial fee of P1,500.00 upon registration and the mandatory fees. Student receives Official Receipt (OR).
DEPARTMENT (COLLEGE):
Enrolment officer evaluates and encodes student’s enrolment details. Student receives hard copy of enrolment form.
SECURITY OFFICE:
Student applies for identification card.
CROSS ENROLLMENT
COLLEGE (DEPARTMENT):
Students must confirm with the department that the subject(s) they wish to enroll in are available. Once confirmed, the department will issue an admission slip, after which you can proceed to the Registrar’s Office to encode their Student Information Sheet (SIS).

The Scholarships office provide university aids and grants and accepts different government and non-government scholarships.
