Find forms for each division, as well as resources related to DESPP. Once you find the forms you need, print them out, and fill out the necessary information. Each form contains directions on where to mail them in or which location to bring them in person. This list helps us assist state and local partners reach as many people as possible with important crisis related messaging. Report email phishing Help reduce cyber-attacks by reporting any suspicious emails. Email phishing will try to trick you into clicking fraudulent links, downloading attachments, or providing personal information. Report suspicious and illegal activity If you see any suspicious activity going on, call 911 immediately. You can also fill out a form with any additional details about the suspicious or illegal activity you witnessed. Every detail helps.
