Participants get support from trained lifestyle coaches to create action plans to make meaningful changes to meet their health-related goals.  These workshops are held primarily online, which connects people from across the state.  People who complete the program share that they appreciate the support of the group and are better able to create new habits with food, physical activity, sleep, and stress management. The Learning to Live Well with Diabetes series was created to share clear, practical information and resources to offer support without adding extra stress. Setting small, manageable goals over time can help relieve stress and lead to lasting health improvements, and you don’t have to do it alone. Your provider may be able to prescribe less expensive medication, or you may be eligible for assistance. The following resources may also help. It provides a summary of recommendations for treatment at medical provider office visits, a review of lab reports, and self-management responsibilities. Insight into Diabetes Care in Vermont From 2018–2023, the Vermont Department of Health partnered with community and clinical organizations to expand self-management programs, strengthen care systems, and promote health equity, impacting over 90,000 adults. This report highlights proven strategies for team-based care, self-management program delivery, health equity and data-driven improvements.
