Summa Health at Home™ provides home healthcare services that give adult patients of all ages the opportunity to heal from illness or injury where they feel the most comfortable – at home. Feeling safe and comfortable is an important part of the healing process. Services focus on individual needs and goals. Our healthcare professionals can provide a smooth transition for you or your loved one from any level of care to a personal home setting or an assisted living facility. Summa Health at Home specialists deliver care to those with chronic conditions and to those recovering from surgery, an injury or a recent hospitalization in the comfort of the patient’s home.

In-Home Healthcare Based on Your Needs
We provide home healthcare based on each patient's medical, personal and situational needs. We work with you and your provider to develop a plan of care to meet your individual needs.

Qualifying for At-Home Healthcare
Summa Health at Home is helpful for patients who find it difficult to leave their homes for medical, physical or psychological reasons.
Other qualification triggers include:
Chronic, severe illnesses such as diabetes, hypertension, pneumonia, stroke or chronic heart failure
Physical disadvantages such as difficulty walking or fear of falling
Multiple medications
Medicare, Medicaid and most major insurance companies cover most home care services, however individual coverage varies.

Family Member/Loved One:
Has problems taking medications or understanding how to take medications.
Requires treatments that need to be done at home, such as checking blood sugar, using oxygen, taking care of a wound or using some type of medical equipment.
Has difficulty walking, falls, is easily tired and does not have enough strength to complete a task.
Has vision or hearing problems, which may present a safety risk.

Feels depressed, anxious or confused so it is more difficult to care for them.
Has difficulty caring for themselves as it pertains to bathing, getting dressed, preparing meals or walking.

Services for Caregivers
Caregivers provide services and assistance to individuals who are too ill or injured to care for themselves. Many times, that person is referred to as a “loved one” because they most often are a family member: spouse, parent, partner, child or sibling. However, the term “caregiver” is universal and includes anyone for whom a caregiver provides care, comfort, support and other services – directly or indirectly.
Caregiving is often a full-time job – if not physically, then certainly psychologically and emotionally. Individuals who assume the responsibility of caring for another person are most successful when they:
Admit caregiving is not easy
Accept help when it’s offered or ask for help if/when they need it
Access community services (many organizations provide valuable services to caregivers as well as to those who need help)
Address their own needs
Recognizing a need to care for the caregiver as well as the patient, Summa Health offers several programs designed to offer support and provide information to those who find themselves in the role of caregiver for a loved one.

Summa Health at Home Outcomes and Quality
Summa Health at Home utilizes several methods to assess quality of care and patient satisfaction.
All programs of Summa Health at Home seek feedback related to care and services provided to patients through the use of customer satisfaction surveys. Following a care episode, Medicare-approved satisfaction surveys are sent to patients and/or caregivers. Several areas of care are explored through targeted questions, such as performance related to weekends and evenings, treatment with respect, communication, rating of care, etc.
Results from these surveys are compiled and trended by a third-party vendor to assure complete confidentiality. Results are also compared to national and Medicare targets. These comparisons assist in identifying areas for improvement.
