Medical Record Requests
Phoenix Children’s has a centralized medical record department with access to all our locations, including inpatient, outpatient, urgent cares and Phoenix Children’s Pediatrics clinics.
The Medical Records office is open Monday through Friday from 8 a.m. to 4:30 p.m. and can be reached via email at

. Health Information Management provides copies of authorization forms that can be completed in person at our Main Campus location.

Requesting Patient Medical Records or Authorizing the Release of Records
All requests for medical records are required to be in a written format per HIPAA guidelines.
Patient and legally authorized representatives (LAR) can request copies of medical records, radiology imaging or itemized billing statements for:
The patient or LAR
Other healthcare providers
Third party entities: schools, military offices, health insurance, etc.
If you are requesting copies of records to be released to an attorney’s office, life insurance company or auto insurance company, please follow the instructions in the Attorney, Life Insurance or Auto Insurance Request for Records section below.
To release medical records, Phoenix Children's requires the permission of the patient or LAR in writing by submitting an authorization form to release protected health information.

Paper copies of the authorization forms are also available at the Information Desk in our Main Campus location.
If you are a patient over the age of 18,
please submit a copy of the completed authorization form and a copy of your photo ID.
If you are the LAR of a minor patient,
please submit the completed authorization form, a copy of your photo ID and any legal documents showing your authority to authorize the release of protected health information. The following are acceptable forms of documentation:
Birth certificate
Medical power of attorney
Adoption paperwork
Court documentation of legal guardianship of a minor/adult
Please note: Health Information Management may request additional information if your relationship to the patient or your authority to authorize the release of the requested records is unable to be verified.
The completed form, along with a valid copy of your photo ID, can be mailed, emailed as an attachment, faxed or dropped off at the front desk of any of our locations.

Processing medical record requests may take seven to 10 business days from the date of receipt. Occasionally delays occur due to high volumes and some requests may take up to 30 days to be processed. Records will be released and delivered in the method indicated on the authorization form (fax, email or Patient Portal). Radiology images will be released through an electronic viewing platform called Nuance PowerShare or sent via secure email.

To avoid faxing to Health Information Management (HIM) or delays in receiving records, HealthPoint is an electronic method for referring physicians to view the records and/or schedule appointments, surgeries, and/or diagnostic testing for a patient.
