Vanderbilt Health is pleased to offer three online options for requesting medical records.
Patient Medical Record Request
For patients requesting their own medical records
For legal guardians or legal representatives requesting records on behalf of a minor/dependent or another patient
Provider Medical Record Request
For health care providers requesting records for continuity of care (e.g., treatment, referrals)
Third-Party Medical Record Request
For attorneys or record retrieval services requesting records on behalf of a client for legal purposes
Additional Information
Requests for medical records by a patient, the legal guardian of a minor child (less than 18 years old), or by a patient’s legal representative may be submitted online by choosing the “Patient Medical Records Request” option above or by accessing the
My Health at Vanderbilt patient portal
To successfully complete your online request, please have the following available:
Mobile phone/tablet
Your photo ID/driver’s license (or a clear picture of your ID that can be uploaded)
Name(s) of provider(s), types of records needed, location, and dates of service for the records to be released
If you are requesting records be sent outside of Vanderbilt Health, you will need the full address, phone number, and if applicable, the name of the provider/facility
If you are requesting medical records about another patient other than your own child, please have the authorizing documents available such as power of attorney, guardianship papers, executor’s documents, birth certificate of the patient, or death certificate of the patient, or other documentation that shows appointment as legal representative

Update Patient Identifiers
Life events or other situations often necessitate updating a patient’s name, date of birth (DOB), Social Security number (SSN) or legal sex in the electronic medical record system. While updating this information during an in-person appointment or visit has always been and will continue to be an option, Vanderbilt Health offers an online option to submit the change request for patients or their legal representative.
Appropriate supporting documentation (government-issued identification or legal document) is required to be submitted in order for the change to be made. To get started, complete the form:
Report a Death
Vanderbilt Health is often unaware when one of our patients dies at a hospital, facility or elsewhere outside of our system. If you would like to make us aware of a patient’s death, complete the online form below.
Please have available the following information in order to fill the form out completely.
Patient's full legal name (and any alternative legal name)
Patient's address
Patient's date of birth (DOB)
Last four digits of Social Security number (or full number), if known
Legal sex
Date of death
Digital copy of death certificate or link to obituary, if available
Alternatively, if you would like to notify us by mail of the patient’s death, send a copy of the death certificate and/or obituary, as well as the patient information details requested for the online form as noted above to the following address:
