Through the use of Results Based Accountability, Livingston County in partnership with Noyes Health, has chosen to focus their 2022-2024 Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP) on the following priority areas, with low
socioeconomic status as their identified disparity to address.
Priority areas include:
Preventing mental and substance use disorders
Healthy eating and food security/Preventing chronic diseases
