DMVA Archives stores official military records for
Colorado Army National Guard
Service members who left service before 2007 and
Colorado National Air Guard
before 2003.
About DMVA Archives
DMVA Archives maintains physical military personnel records for individuals that served in the
Colorado Army or Air National Guard
, before the transition to electronic file keeping systems. The information available in a military personnel record includes:
DD-214 Certificate of Release or Discharge From Active Duty
NGB-22 Report of Separation and Record of Service
NGB-22A Correction of NGB-22
NGB-23 Retirement Points Record
NGB-0122 Federal Recognition Appointment Order (Officers Only)
20 Year Retirement Letter (National Guard)
DD-4 Enlistment Contract

How To Request Your Record If You Separated Before 2007
Download, fill out, and sign a records request form,
Standard Form 180 (SF180)
Submit your completed SF180 and a legible clear copy of your government issued photo ID with a date of birth on it to the DMVA Archives. If we cannot read your ID, you will be asked to resend a legible copy. If the ID does not have a date of birth on it, you will be asked to send a different form of ID.

If you mailed in your SF180 but forgot to include a copy of your photo ID, you can
submit just your ID online using this form
DMVA Archives Does Not Have Your Record If:
you left the Colorado Army National Guard after 2007. Please submit
an SF180
and photo ID to the G1 Personnel Office: SSG Filter, Stefanie

Please note that the G1 receives a large number of requests for records daily.
The G1 will work your request in the order that it is received.
Please do not send multiple emails or call multiple times. This may slow down the process.
you left the Colorado Air National Guard after 2003. Please contact the
Air Reserve Personnel Center (ARPC) online
or at 800-525-0102.
you served in another state's National Guard you will need to contact that state to get your service records.
you are looking for records related to serving in another branch of service, DMVA Archives would not have that record either.
you served on Active Duty or as a Reserve in the United State Army, Navy, Marine Corps, Air Force, or Coast Guard, DMVA Archives does not have your record. To request your records, visit the
National Archives online
or call them at either 314-801-0800 or 1-866-272-6272 (Toll Free).

Regardless of if you think DMVA or another organization has your record, you can reach out to your County Veteran Service Officer (VSO) if you need assistance submitting a request or locating your record. For Colorado County VSOs, please visit the

Employment Verifications and Background Checks
DMVA Archives cannot process employment verifications or background checks. This includes requests related to litigation.
To verify a Soldier or Airman, you will need to

To verify a Colorado Department of Military and Veterans Affairs employee, contact the DMVA Human Resources office at 720-250-1520.
Historical Documents or Information
DMVA Archives only manages military personnel records and does not store any documents pertaining to the history of the Colorado National Guard or its members.

Medical Records
Locating the entirety of your medical records can be tricky since portions may reside in several different locations. It is recommend to contact the CONG Medical Office as they are proficient at locating and procuring medical records.

National Archives
Individuals or family members may submit a Standard Form 180 (SF180) to the National Archives to request DD214s and other service records. The SF180 and more information is available on
the National Archives
If additional access to military records is needed, a Freedom of Information Act (FOIA) request can also be submitted
to National Archives on their website
Hours
Our business hours are 7 AM to 5 PM Tuesday through Friday.
We are closed on Mondays.
The Archivist can pull records on Tuesdays and Fridays. If you submit a request you can expect to hear back by phone or email the following Tuesday or Friday if we have your record and how long processing will take.
