Physicians and Practitioners
Paid Family Leave (PFL) provides benefits to people who need time off work for family leave. If a family member submits a PFL care claim, you must complete the DE 2501F Part D – Physician/Practitioner’s Certification. This certifies your patient’s illness or injury and provides the reason for care.
You can complete your medical certifications online or by mail.
To certify online:
Create a
myEDD
Set up your SDI Online account.
Once both accounts are set up, log in to myEDD and select
SDI Online
Use
SDI Online
to:
Find the Care Provider’s claim for benefits.
Submit a claim certification.
View your claim certification history.

You must certify by mail. Complete the

. Licensed physicians or practitioners should not complete this form.
