Housing and Homeless Mental Health Outreach
Safe and stable housing is key to recovery.
Housing is vitally important to living well with a serious mental illness and preventing hospitalization. We’re here to help clients find housing that fits their needs and provide outreach to those experiencing homelessness. While these programs don’t provide immediate emergency housing, we’re here to help you navigate options and the application process.
Supervised Group Living
Often a step-down from hospitalization, Staff are on-site 24/7 to help clients transition back to the community. These homes offer shared living space.
Transitional Living Facilities
These homes are open to clients ready to engage in supportive services, with a primary goal of working toward permanent housing. Staff are on site Regularly for individual and group work.
Permanent Supportive Housing
These apartments prioritize housing stability, and residents can engage in wraparound services as they’re ready. Clients enter this housing through the Coordinated Entry process.
Independent Housing
We help clients obtain housing through Private rentals across Elkhart and St. Joseph counties. Engagement in services is not required, though support is available and encouraged.
Rental Assistance
We can help with rent assistance for people who are experiencing homelessness and also have a mental illness or substance use disorder. Oaklawn clients should ask their provider for a referral.
Each of these housing options has specific requirements. Clients can ask their provider for a referral to Oaklawn’s Housing Services.
PATH Outreach, Serving Elkhart and St. Joseph Counties
The PATH program (Projects for Assistance in the Transition from Homelessness) provides outreach, engagement, and services to people who are experiencing homelessness and have a mental illness and/or a substance use disorder. PATH specialists can help people access Coordinated Entry, attain emergency shelter, apply for housing, and connect to mental health treatment, government resources, and community partners. While PATH does not directly provide housing, outreach specialists provide the supportive services to guide individuals to stable housing. For more information, call

Visit one of our PATH walk-in times to speak to a PATH Outreach Specialist. You do not need to call to schedule an appointment. Visit during any of the days/times listed below at the location convenient for you. Walk-in locations and hours are:
Goshen
Mondays 10 a.m.-1 p.m. – The Window, 223 S. Main St.
Wednesdays 10 a.m.-1 p.m. – The Window 223 S. Main St.
Elkhart
Mondays 1-3 p.m. – Oaklawn, Elkhart campus, 2600 Oakland Ave.
Tuesdays 11 a.m.-3 p.m. – Guidance Ministries, 216 N. 2nd St.
Thursdays 10 a.m.-noon, Oaklawn Elkhart campus, 2600 Oakland Ave.
Thursdays 1-3 p.m., Guidance Ministries, 216 N. 2nd St.
South Bend
Mondays 8:30-10 a.m.-Broadway Church, 1412 Carroll St.
Tuesdays 12-2:30 p.m.- South Bend Library, 304 S. Main St. (room L215)
Wednesdays 10 a.m. -2 p.m.- Oaklawn, South Bend campus, 415 E. Madison St.
Fridays 8:30-10 a.m.-Our Lady of the Road, 744 S. Main St.

City Outreach Team, Serving South Bend
The City Outreach Team (COT) is designed to support people who are unstably housed in South Bend. Peer Support Professionals can help people access Coordinated Entry, attain emergency shelter, help with housing applications, connect to mental health or substance use disorder treatment and access government resources and community partners. COT does not directly provide housing but helps with housing applications. For more information, call
