How to Request Records By Mail or Fax

2. Pay Fees
Before submitting your request, you will need to:
Pay any state-required fees by calling the Medical Records Department
(915) 242-8575
Show proof of identity with a government issued photo ID
Fax the completed form to (915) 242-8574
Requesting copies of your medical records for continued care can be mailed or faxed directly to the physician without charge.
3. Mail Your Request
Your request will be processed and mailed to the address on the authorization form, or you may call to arrange pick up. Please allow a minimum of 3-5 business days from the date we receive your request for us to process it.
