To participate in athletics at SRDS in grades 5-12, you will need to complete the following steps:
1.  Register online with Family ID via the link below.
2.  Print out the forms below and bring them to the physician conducting the physical.
3.  For all athletics, completed forms must be submitted to Nurse Lei Ann McChesney (
lmcchesney@saddleriverday.org
).
