Incident Reporting Forms
Do you need to report an incident?
We are here to help.
Please determine which reporting form below relates most closely to the main reason you are reporting.
All forms allow for anonymous reporting, not just the silent witness form
All forms are typically reviewed within 24-48hrs
Photos, Videos, and other documents can be attached to reports

Information provided in a report will be kept private and shared with only the administrators and individuals needed to address a situation. For more information on what happens after a specific report, click the appropriate link below.

Anonymous reporting may limit the university's ability to follow up on a report. When a person is identified in a report and is a member of the university community, a University official may contact that individual to schedule a meeting to discuss available resources, gather more information, or address information provided in the report.
You can receive a copy of your report as long as you provide a valid email address. Besides a copy of the report, you will also receive information regarding resources, policies, and administrators you may wish to contact.
If you choose not to provide an email address because you wish to report anonymously, it is recommended that you copy the text of your report before submitting it.
You may be contacted about the information you have entered into the form as long as you provide contact information. An individual who has been identified as being targeted by alleged behavior, being a witness to an incident, or is listed a student of concern, when identified, will also be contacted in most instances.
