SCHEDULE A TOUR
ADMISSIONS: FREQUENTLY ASKED QUESTIONS
Commonly referred to as MCH, Monroe Community Hospital is a licensed skilled nursing facility providing short- and long-term care, including rehabilitation, pediatric care, ventilator services, memory care, and palliative and hospice care. MCH serves Medicare and Medicaid recipients and is owned and operated by the County of Monroe.

Patients on the rehab unit are given an enhanced dining experience. A menu is given daily and patients can choose items from the menu. Food is served restaurant-style and patients are encouraged to eat in our dining room.
The admissions process includes a two-step review to evaluate specific medical needs and complete a financial review.
The clinical review is completed by our Admitting Coordinator and/or Clinical Admissions Coordinator in close collaboration with our clinical staff to determine whether MCH is the appropriate level of care.
We review standardized assessments and medical records and, in some cases, conduct interviews to ensure individuals meet the criteria for skilled nursing care. Each referral is carefully evaluated to confirm we can meet the individual’s medical, psychosocial, and care needs.
The financial review is conducted by our Financial Coordinator in collaboration with the admissions and clinical admissions teams. Each referral is reviewed to verify insurance coverage and payer source. This process may include discussions with family members, insurance representatives, referring facilities, attorneys, and the MCH Business Office.
Once both the clinical and financial reviews are complete, our team makes a final admissions determination. Beds are offered based on availability at the time the referral is reviewed.
Timing varies based on referral volume and bed availability. MCH works with all major health systems in the Monroe County region to ensure an efficient admissions process; however, referrals from home or other community settings may take longer to allow time to collect all information needed for a comprehensive review.
MCH strives to provide timely communication. If there is a delay, you may contact us to inquire about a specific referral or to address any additional questions.
MCH works with all major health systems in Monroe County, surrounding areas, and, in some cases, outside the region. To begin the referral process, please contact your unit social worker and request a referral to MCH.

. Along with the completed application, the following information is required:
PRI/screen completed within the past 30 to 90 days
Most recent history and physical from a primary care provider
Current immunization records
MOLST form
Health care proxy, power of attorney, living will, or guardianship documents, if applicable
PRI stands for Patient Review Instrument, a standardized assessment required by New York state prior to admission to any skilled nursing facility. It is completed by a registered nurse or other qualified individual and is used to evaluate care needs and determine the appropriate level of care.
If a referral originates from a hospital, the hospital completes the PRI. If the referral comes from home or another community setting, agencies are available to assist with completing the PRI. Please contact us if you have questions or need assistance obtaining one.
MCH is licensed for 566 beds, including sub-acute rehabilitation services, 24-hour skilled nursing care, and specialty units for respiratory needs, pediatric care, dementia care, and palliative and hospice care. Bed availability changes daily, with offers based on availability at the time of referral.
MCH works diligently to provide services in the most cost-effective manner possible. Like most health care providers nationwide, we continue to experience rising health care costs. Despite these challenges, MCH remains committed to delivering exceptional care.
As a result of legislation enacted in 2021 by New York State, nursing homes are required to post certain information on their websites to ensure public accessibility. This information includes the maximum rates charged for residency and services, which are listed below.
Monroe Community Hospital regularly enters into contracts with vendors for goods and services necessary for the normal operation of the facility. The largest expenditures are with vendors providing medical and physician services, medical supplies, food and dietary services, and environmental services.

MCH is owned and operated by the County of Monroe, which also owns the hospital grounds and facilities. The daily rates for skilled nursing went into effect on January 1, 2026, are as follows.

The Admissions Department receives approximately 300 to 400 referrals each month. Each referral is thoroughly reviewed based on clinical needs and bed availability.
