How to access your medical records
Access records in MyChart
Access records in MyChart
You may access and request a transfer of your medical record information quickly and electronically with a MyChart account.
You may also sign up to view your child’s or another adult’s medical records through MyChart by visiting the MyChart Forms website (see link below) and completing the appropriate application form.
Please note, medical records from Medical Center Downtown (formerly Mercy Iowa City) prior to May 4, 2024, and Mission Cancer + Blood prior to June 2, 2025, are not yet available in MyChart. The transfer of these medical records to the UI Health Care MyChart Patient Portal is still in progress. The completion date for this documentation transfer is unknown at this time.

Effective 1/30/2025, access to FollowMyHealth has been discontinued. Medical records for visits at Medical Center Downtown between 3/18/2022 and 5/4/2024 can be accessed by either submitting a written request or by calling 319-356-1719. The request form and additional information can be found below. Care received on or after May 4, 2024, is accessible through MyChart.
The preferred method to receive medical record requests is in writing or via MyChart. However, when these options are not available, requests may be sent via e-mail or fax.
