Goals

By 2027, stable revenue streams that sustain integration across all of Hope Clinic. Increase to $4 million annually and becomes $8 million in client support.

By 2027, services are integrated between programs (Medical, Dental, Food, Behavioral Health). No matter what your initial point of entry is, you have access to the care you qualify for seamlessly.

By 2027, Behavioral Health will integrate into each program systems of detection, treatment, and/or referral for behavioral health and targeted SDOH to promote whole person care. Behavioral Health will demonstrate patient outcomes as a result of services.
