The Health of People in Vermont
What do we know?
The
Vermont State Health Assessment
is an overview of what we know about the health of people in Vermont at a point in time. It uses data to understand health outcomes and differences across groups. The Assessment is conducted every five years.
What are we going to do about it?
Results of the State Health Assessment are used to develop the
State Health Improvement Plan
. The Plan is a five-year roadmap for ensuring that all people and communities in Vermont have access to opportunities for health and well-being. It reflects the shared efforts of people and organizations across the state. It outlines what we want to achieve, how we will achieve it, and how we will know if we are making a difference.
2025-2030 State Health Improvement Plan
There are four goals in the plan related to housing, cost of living, access to care, and mental health and substance use.

Help Vermont achieve its health goals!
There is a lot of great work already happening across Vermont that contributes to achieving the goals of the State Health Improvement Plan. We invite organizations leading this work to be partners in implementing the Plan. This is a unique opportunity to highlight your work and help tell the story of how we are working to achieve our state health goals.
Updates on implementing the Plan will be shared in annual reports available to the public, legislators, and other interested groups. The Plan is also a tool for partners to guide improvements to their programs or advocate for policies and resources that support their work.
There are different ways to be an implementation partner. You can choose what works best for your program or organization:
Share an activity you’re already doing:
Include one or more activities underway at the organization that can be highlighted in the State Health Improvement Plan.
Share data for performance accountability:
Include one or more program performance measures you are tracking and could report in a public dashboard.
Champion or implement a new activity:
Identify a new activity you would like to lead, such as implementing or improving a program or advocating for a policy that supports the state’s goals.
Join a workgroup:
meet monthly to collaborate with and learn from other state agencies, community organizations, and individuals with lived expertise around successes, challenges, and opportunities in implementing the State Health Improvement Plan.
