Student Absence Form & Attendance Excuses
Student Absence Reporting Form (Always Required)
If a student is absent for any reason, this form must be completed by a parent/guardian. We ask that it be completed by 8:30 am on the day of the absence to allow our administrative team time to properly account for all students. It may even be completed in advance when planned absences are upcoming.
