Anticoagulation clinic services
The Anticoagulation Clinic is managed by clinical pharmacists who work closely with your provider. Although blood thinners are an important part of a long-term care plan for many patients, they require careful monitoring.
Our state-of-the-art blood test uses a small finger stick to measure how fast your blood clots and your anticoagulation pharmacist will adjust your dose as needed to keep you in therapeutic range.
If you are taking warfarin, please ask your provider for a referral to the Anticoagulation Clinic, located at the Celilo Infusion Center.
Medication therapy management
Our pharmacists review medication lists across the primary care and specialty clinics to check treatment plans and watch for medication safety and effectiveness.
