UI Health Media Photo Consent Form
before their image may be captured or used for any internal or external communications, including social media.
Employees may provide verbal consent
for photography and use of their image; a written Media Photo Consent Form is not required for employees.
If there is any uncertainty, do not proceed with photography until proper consent is obtained.
