Palliative Home Care
Our
palliative home care
team provides a critical option for patients who prefer to and can be cared for at home. Our doctors, nurses and social workers make home visits to care for patients and support families by helping them manage the symptoms of advanced illnesses. This includes providing training and emotional support to their caregivers. Our palliative care team is contactable on the phone, 24 hours a day, to address any urgent concerns.
FAQs for Palliative Home Care Service
What does the palliative home care service entail?
Our team of doctors, nurses and medical social workers work together to support you and your family in caring for your loved one at home.
A nurse will be assigned to your loved one to make home visits. The frequency of the visits will be based on the condition and needs of the patient. If the patient is stable, we will visit at least once a month. If the patient is symptomatic, we will visit more frequently. Doctors or social workers will visit as needed.
Can I reach your service before the first visit is made?
Yes. When we first contact you via phone, you will be given a contact number that you can use to reach us during office hours from 8am-5.30pm, Monday to Thursday and 8am-5pm on Friday.
If my loved one is still in hospital before the first visit is made, what should I do?
It’s important that you let us know once your loved one is discharged from hospital. You can reach us during office hours at the number that was given to you. We may also contact you intermittently to check on the discharge date.
I heard that your palliative home care service is 24hrs. Is it true?
Yes. It is true. Our after-hours on-call service is only available after we have made the first visit. Should your loved one require medical attention before that, please bring him/her to the hospital or General Practitioner.
Do I need to pay for the palliative home care service?
No, our palliative home care service is free of charge to patients.
