To arrange billing, removal, transfer, or disposal of any material stored in the Geological Materials Repository Geological Materials Working Collections, please see applicable forms below. If you are uncertain of which forms you need to complete, please contact the GMR staff for clarification.
Forms
"Webforms" forms are available to USGS employees from the internal-only website.
Disposal
Webforms DI-103A V.2 is the DOI required form for disposal of any DOI owned material.
GMR disposal form -- For material approved for disposal please fill out one GMR Material Disposal Form per pallet, return to the GMR Staff, and arrange for payment of the \$75 fee.

Transfer
Transfer material that remains in GMR to another USGS entity:
GMR Internal transfer form-- For material within GMR approved for transfer to another USGS entity please fill out one GMR Material Transfer Form - Internal per pallet, return to the GMR Staff, and arrange for payment of the \$75 fee. This is for transfer of material in the GMR from one responsible steward to another

Webforms 9-064-USGS required form for transfer to another USGS entity.
Transfer to another DOI entity:
Webforms SF-120—required by USGS for transfer of USGS material outside of USGS.
Webforms DI-104--DOI required form for transfer within DOI of DOI owned material.
GMR external transfer form-- For material approved for transfer to an outside entity please fill out one GMR Material Removal Form - External per pallet, return to the GMR Staff, and arrange for payment of the \$75 fee.
GMR Material Transfer Form - External
Transfer to another federal agency:
Webforms SF-120—required by USGS for transfer of USGS material outside of USGS.
Webforms SF-122—required by USGS for transfer to another federal agency outside of DOI.

Donation
Webforms DI-103A V.2 (Certificate of Unserviceable Property) if donated to a public entity in lieu of destruction.
OR Webforms SF-120 and Webforms 9-064 for donation of collections used for scientific and/or technological training or research pursuant to the American Technological Preeminence (Stevenson-Wydler) Act required by USGS.
Removal from GMR
For a pallet of material approved for removal, please fill out one GMR Material Removal Form per pallet and return to the GMR Staff and arrange for the payment of the \$75 fee.
GMR Material Removal Form
After removing from GMR, if the material will be transferred from one USGS entity to another, the Webforms 9-064-USGS form is required.
After removing from GMR, if the material will be transferred to another DOI entity:
Webforms SF-120—required by USGS for transfer of USGS material outside of USGS.
