We love our volunteers. It takes time, energy and sometimes patience, but volunteering makes a difference for our students and our LLA community. Volunteer Packet for New Volunteers If you are a new volunteer please complete the entire volunteer packet and submit to your division office. Volunteer Instructions Instructions for how to become and maintain clearance to be a volunteer at Loma Linda Academy. Volunteer Application and Commitment The volunteer commitment and application need to be completed and turned in annually. Livescan Form Receive fingerprint background check at approved location and then provide the school copy of Live Scan Receipt. (Only required one time unless a new background check is requested). TB Risk Assessment Questionnaire Tuberculosis (TB) Risk Assessment Questionnaire – required every 4 years. TB Test Report - ONLY required if you answered “Yes” to any question on the TB Screening Assessment Questionnaire. Make sure to complete both front AND back pages of the questionnaire. Mandated Reporter Training Must be completed every 2 years Complete the California state-mandated reporter training for volunteers. Website to complete training: Login or create a new account under ‘Start Now’. Fill out information and enter ‘Education’ for Industry, ‘Volunteer’ for Profession, and ‘Loma Linda Academy’ for Organization Name. Once logged in, under ‘My Training’ click ‘Add Training’ and select Volunteers | CA, Mandated Reporting of Child Abuse. Submit the certificate of completion to the school: Print a copy to turn in with your completed volunteer application packet Cost: $8.99 (paid directly on the website) Volunteer Vehicle Information Form Please submit a new vehicle information form if your vehicle has changed. Driver's Record Request - CLICK HERE To be able to drive students at LLA you will need to obtain copy of driver's record from the DMV website for $2. Along with providing a copy of a valid driver's license, vehicle transportation information, and a current car insurance card.
