Requesting MyBeacon access for a family member or child’s account
For children, giving access to an adult to view their MyBeacon account is called giving “proxy” access. A person with this access is able to view the MyBeacon medical record for the patient, including test results and doctor’s notes.
Ready to request access? Choose the scenario that best describes your request. I need MyBeacon access for:
Myself
You’ll need to provide:
First name
Last name
Date of birth
Phone number
A child aged 0-13 years old
Patients between the ages of 0–13 cannot create their own MyBeacon account, so an adult (parent, guardian, etc.) needs to request proxy access to the child’s records. This adult would then be granted full medical records access. (Due to legal and privacy regulations, foster parents are not granted access to a foster child’s patient portal.)
To request access, review and complete the
consent form
. Access will be provided within 24-48 hours (about 2 days) and the adult will receive a welcome email when it is ready.
If you have a MyBeacon account of your own, we suggest you use your existing portal email address so you can access the proxy account from your login.
If you are a Beacon patient but don’t yet have a portal account, we suggest setting your personal portal account up first before requesting proxy access to your child’s account.
If you don’t have a MyBeacon account, you can still receive proxy access to your child’s account.
A child aged 14-17 years old
Individuals between the ages of 14–17 cannot create their own MyBeacon account.
State laws give minors the right to seek care on their own for some important health conditions. Minors also have the right to limit access to conversations they have with doctors about sensitive health-related topics. Minors might not talk about these topics if they think information could be shared.
Beacon Health System protects the privacy of all patients, including minors. Sometimes, minors have concerns they want to or are legally able to talk about privately and/or independently with their healthcare providers. Beacon has a policy to allow for these unique situations according to state laws.
Because of these rights, access to MyBeacon for teens aged 14-17 can only be granted at your Beacon Medical Group office or Health Information Management office at any Beacon hospital. Both a parent and the teen must be physically present to discuss this access with Beacon staff, agree to it and sign the consent form for access:
Patient Portal Consent (Age 14-17) Form
(English) or (
Spanish
). This form cannot be completed and submitted online.
More information about proxy access for this age group:
If granted, proxy access for teens aged 14-17 grants full medical records access, including results and notes to the parent/guardian.
When medically necessary, a doctor may use their clinical judgment to disclose health information about a minor to a parent or guardian to prevent a serious and imminent threat to the health or safety of a minor or another person.
This proxy access expires on the teen’s 18th birthday.
The teen may revoke the parent’s access at any time and the parent may waive their right to access at any time.
Indiana and Michigan state laws permit minors to seek treatment without parental consent for:
Testing and treatment for sexually transmitted infections.
Testing and treatment for pregnancy and the prevention of pregnancy in certain situations.
Drug and alcohol treatment.
Another adult
Patients 18+ may grant another adult proxy access using the required form below. Proxy access means that the other person would have access to the patient’s full medical records. This access does not expire but may be revoked by the patient at any time.
To add a proxy to your MyBeacon account, review and complete the
consent form
. Access will be provided within 24-48 hours (about 2 days) and you will receive a welcome email when it is ready.
If you have a MyBeacon account of your own, we suggest using your existing portal email address so that you can access the account from your login.
If you are a Beacon patient but don’t yet have a portal account, we suggest setting your account up first before requesting proxy access.
If you don’t have a MyBeacon account, you can still receive proxy access.
Revoking others’ access to MyBeacon
Some office visits may be for very sensitive issues. We cannot stop specific test results from showing on MyBeacon. This includes tests for pregnancy or sexually transmitted infections.
You can ask your doctor not to post certain information in your notes to MyBeacon, but you cannot restrict results from posting to your portal. Talk to your doctor about concerns about your information in MyBeacon.
You have the right to take away MyBeacon access from anyone you gave it to. We can help you remove someone from your MyBeacon account if you need assistance. You will use a form available from our medical records office, your doctor’s office or by completing and returning the
Patient Portal Revocation Form (English)
or
