Complaint and Reporting Process
Please use this form should you find yourself in conflict with a University process or policy—academic or non-academic—or if you are dissatisfied with the delivery of services provided by a University faculty or staff member.
This form is NOT for issues related to
grades,
the Americans with Disabilities Act,
or discrimination, harassment, sexual misconduct, or retaliation under Title IX.
Issues related to grades follow the
Grade Appeal Process
outlined in the current University catalog, and issues related to the Americans with Disabilities Act, or discrimination, harassment, sexual misconduct, or retaliation complaints should be reported using the process outlined in the
Student Handbook
.

Central State University encourages victims of sexual misconduct, discrimination, and harassment to talk to somebody about what happened so they can get the support they need and so that CSU can respond appropriately. Though CSU will keep reports as confidential as possible, the University cannot guarantee the confidentiality of every report or complaint.
