Student Insurance
Students who are admitted to a degree program and are registered for six or more credits per semester that count toward the automatic assessment of the Student Services Fee are
required

UPlan members
may choose Boynton as their primary care clinic; consult your insurance card for details. Flu shots and health screenings are offered to UPlan members at no out-of-pocket cost. These benefits also earn Wellness Program points to lower UPlan premiums. See details about the UPlan on the

Co-pays
Co-payments are to be paid at the time of service. We will invoice you directly for any co-payments required by your insurance. The University-sponsored
Graduate Assistant Health Plan
requires co-payment for services.
Preventive and Non-preventive Care Costs
Your annual physical or routine wellness checks are considered preventative care visits. The purpose of the visit is to supply your provider with medical history information and prevent health problems before you are sick.
Preventative visits typically include:
Physical exam (breast/pelvic if needed)
Vaccines (Flu, COVID-19, HPV up to age 26, Tdap, Smallpox, MPox, and Meningococcal)
Screenings (i.e. screenings for sexually transmitted infections, for breast, cervical, colorectal & prostate cancer, and screenings for diabetes & high cholesterol, depending on risk factors)
Medical and screening reviews
Preventative health coaching
If your visit includes non-preventative services, you may be responsible for a copay, coinsurance, or deductible.
Non-preventative services typically include:
Physicals that require forms to be filled out (i.e. sports physical or admission to an educational institution)
Evaluating, diagnosing, or treating a new or existing condition (i.e. a sore throat, chronic migraine, or knee pain)
Lab testing or medication refills for a new or previously diagnosed condition
If you have any questions about your preventative care benefits, please contact your insurance company’s member service department. The number can be found on the back of your insurance card.
Student Service Fee
With some exceptions, the
Student Services Fee
(SSF) helps cover out-of-pocket costs of care at Boynton for students with active health insurance and who have paid the SSF.

Student Services Fee Limitations
SSF benefits only apply to care at Boynton Health for students with active health insurance coverage. Check with your health insurance provider about coverage for care at other locations.
The SSF can only be applied during the semester it is paid.
Billing and the Student Services Fee
After billing your health insurance and applying your SSF to the cost of your visit, you can expect most Boynton services at no additional out-of-pocket cost — with some exceptions.
Not covered by the Student Services Fee
Prescription medications
Medical supplies
Vaccines (except HPV for ages 9-26, flu, COVID-19, Meningitis, Mpox, and Tdap, which are covered)
Contact lens fitting and follow-up care
Eyewear: glasses, frames, lenses; contact lenses, etc.
Physical therapy (co-pay)
Physical examination for the completion of a form

How Billing and Student Services Fees Work
After your visit, we will bill your health insurance for the costs of services. Any amount not covered by your health insurance will be billed to you at a later date.
If you are a student who pays the Student Services Fee, the amount of your bill not covered by insurance will be paid for from the Student Services Fee—some exceptions apply.

Is my insurance taken at Boynton?
Students who have active health insurance coverage and pay the Student Service Fee receive most services at Boynton at no or reduced cost.
Can I be seen at Boynton if I am a fee paying student, but do not currently have insurance?
You can be seen at Boynton, but you would be responsible for ALL charges and your student service fees would NOT apply.
How do I find out if my insurance covers a service?
Contact your insurance provider to learn more about your coverage.
What is an explanation of benefits or an explanation of payment?
An Explanation of Benefits (sometimes called an EOB or an explanation of payment) is a statement your insurance company sends that summarizes the costs of health care services you received. It shows how much your insurance company is paying your healthcare provider and how much you may be responsible for paying. This is not a bill, but you will receive a statement from Boynton if you owe anything not covered by your paid Student Services Fee.
Why is there a charge on my student account?
After two statements have been sent and there is no payment, it is our policy to transfer the balance to your student account. You will see these charges as ‘BHS Medical Charge Transfer” on your student account. This policy is stated in our billing statements.
Why did I receive a bill from Boynton?
You may have received a bill for one of the following reasons:
Boynton does not have your insurance information.
After Boynton billed your insurance, there may be a charge (or a portion of the cost) that your insurance did not cover, and it is one of the several items that the Student Services Fee (SSF) or Extended Coverage (EXT) does not cover.
Boynton received a denial or rejection for Coordination of Benefits from your insurance provider. In this case, you will need to reach out to your insurance provider, update your information, and ask that they reprocess any denied claims.
If Boynton has your insurance information and you do not have SSF or EXT, you are responsible for any co-pays, deductibles, and/or coinsurance.
How can I pay my Boynton bill?
You can mail in a check, or complete the credit card information on your statement and mail that in. Or you can drop off a payment at the Cashier’s Office on the 3rd floor. You can also call Patient Accounting at
612-624-6985
to pay with a credit card over the phone. Online bill payments are not available at this time.
How do I pay a bill that I received from Quest Diagnostics, Inc.?
If you have paid the Student Services fee, contact Patient Accounting at
612-624-6985
and have the service date and invoice number ready. We will contact Quest, and you should not receive another bill. If you do receive another bill, please contact Patient Accounting again.
The bill is your responsibility if you have NOT paid the Student Services fee.
If Boynton refers me to another provider or facility will my Student Services Fee cover costs there?
No, the Student Services Fee is only applicable at Boynton. Any costs from other healthcare providers or facilities are your responsibility.
